BizActions has created a series of typical types of consent needed for various industry segments.  You may include these Consent Types with the default text, edit the default text, or create your own custom Consent Types and text.

As you can see on the Consent Types below for the various segments, selecting to include a checkbox for a Consent Type will add the checkbox to your consent form, making it easy for your recipients to respond.
